 have a BSA health form. Health forms and troop rosters must be furnished 
 upon check-in on Sunday afternoon - or they will not be allowed to stay o
 n the Reservation.  \n​\nThe health form must be filled out completely\, 
 including the following\, listed below:\na. Parental Signature - For thos
 e under 18 years of age\, this gives us the approval for emergency treatm
 ent or surgery in the case of an accident or serious illness.\nb. Doctor’
 s Signature - Each Scout and adult must have a medical exam given within 
 12 months of going to camp. No examinations will be given at camp. Many h
 ealth care offices will only provide a printout from their office rather 
 than fill out part C of the BSA health form.\nThis form must include a Do
 ctor's Signature.\nThis form must certify all information of part C on th
 e health from.\nFollowing the criteria listed\, this form is acceptable a
 s a substitute for the Doctor filling out that potion of the health form.
 \nThis can be stapled to the BSA form.\nc. Health and Restriction Informa
 tion - Include all information relating to the individual’s health\, incl
 uding medications\, restrictions\, allergies and other pertinent informat
 ion.\n\nd. Health Insurance Information - Be sure to include the insuranc
 e carrier and policy number on the health form.\ne. Vaccination Informati
 on - A current (within 10 years) tetanus shot is necessary and MUST be no
 ted on the health form.\n\nf. If a Scout or leader has an examination for
 m from another source\, we will accept it as the examination portion only
 . Fill out a standard health form completely\, with the exception of the 
 physician’s portion\, and attach to the bottom of the other form. This al
